Stomal Therapy
The journey with a Stoma or “Ostomy” may initially feel overwhelming and challenging. It doesn’t need to be.
Initial Consultation

- Activities of daily living
- Complete ostomy appliance bag change, assessment,
and reapplication of appliance
- Education
- Recommendations
- Assistance with ordering of new supplies (if applicable)
- Report to GP (client consent)

Wound Care
Initial Consultation:
-Presenting concern from client (goals)
- Medications
- Allergies
- Past medical history
- Diet and fluid intake (nutrition and hydration review)
- Bowels or bladder pattern
- Activities of daily living
- Skin assessment
- Wound assessment including wound cleansing,
measurement, wound product recommendations, wound
dressing application, wound dressing plan, ordering of
supplies (in consultation with case manager), frequency
of wound dressing changes
- Education for client/carer/family
- Recommendations for further investigations (if
indicated)
- Report to GP (client consent)

Scope & Limitations
This service specializes in managing complex, hard-to-heal wounds, including:
- Chronic wounds
- Malignant wounds
- Fistulas
- Pressure injuries
- Acute wounds
- Skin tears
- Diabetic foot ulcers
Key Services:
- Comprehensive wound assessment and individualized care planning.
- ABPI readings to assess arterial circulation, with referrals to vascular specialists if needed.
- Comprehensive education for clients, families, and healthcare professionals on wound care best practices, prevention, and management.
- Hands-on clinical training – If healthcare professionals or organisations require face-to-face practical training in a clinical setting, please contact Emily to discuss arrangements.
- Collaboration with medical practitioners is preferred to ensure continued coordination, updates on wound progression, and to determine if further investigations such as wound swabs, blood tests, or scans are needed.
Virtual consultations – Available for healthcare professionals, carers, or patients, although these may limit the visual ability to provide the same level of care as face-to-face consultations.
- Holistic strategies to support healing and reduce complications.
Unavailable services:
- Compression therapy for venous leg ulcers.
- Burns management.
This service ensures high-quality wound care while facilitating specialist referrals and medical collaboration when necessary.

Continence (Adult)
Initial Consultation:
- Presenting concern from client (goals)
- Medications
- Allergies
- Past medical history
- Diet and fluid intake (nutrition and hydration review)
- Bowels or bladder pattern
- Activities of daily living
- Skin assessment
- Current continence aid products used
- Recommendation of sample continence aid products to trial (order samples to be sent to client), adjust samples if needed once trialled. Once continence aid product suitable, add to final report for obtaining supplies through funding
- Education to client/family/carer for day to day modifications of diet/fluids/toileting techniques.
- Report inclusive of other products that may be of benefit such as mattress protector (disposable and washable, skin hygiene wipes, barrier creams, foot stool for optimal toileting positioning)
- Report to GP (client consent)
Prior to first consultation, if client is able to (if appropriate) complete a 7 day bowel and bladder diary, this would be of benefit to assist in the consultation.

Scope & Limitations
This service focuses on holistic health assessments and basic interventions for bladder and bowel incontinence. It includes:
Comprehensive Health & Continence Assessment – Evaluating medical history, symptoms, lifestyle factors, and continence-related concerns.
Basic Interventions – Providing education, lifestyle advice, bladder/bowel training, and continence product recommendations.
On-Referral to Specialists – For advanced care needs, referrals are made to:
Pelvic Floor Physiotherapists – For targeted pelvic muscle rehabilitation.
Continence Nurse Consultants (CNCs) – Specializing in intermittent self-catheterization, indwelling catheter (IDC), and suprapubic catheter (SPC) management.
Urologists or Specialists – For uroflow studies and further diagnostic assessments.
This service does not provide complex interventions but ensures individuals are assessed and referred appropriately for specialised care.

Must have a GP referrals to ensure clear parameters for clinical escalation and deterioration in consultation with GP.
