  • Includes a general health assessment + health history review

  • Stomal Therapy: + Assessment + personalised management plan

  • Wound Care: + Assessment + tailored management plan

Step 2 – Follow Up

  • Conducted via telehealth

  • Scheduled two weeks after the initial review

  • Ensures the management plan is effective and adjustments are made if needed
